April 14th, 1964, a delta rockets 3rd stage prematurely ignites in the assembly building at the Kennedy Space Center, killing 3 people.
Also on 1981, Space Shuttle Columbia on the mission STS-1 lands saftly after it's orbital mission.

Touchdown of the two landing gears, the nose gear is still waiting for touchdown.

Pilot Robert Crippin walks out of the shuttle on to the ground, Commander John Young is waiting for him at the bottom of the steps.

Also, it's Valentin Lebedev birthday, the Russian Cosmonuat flew on Soyuz 13 and Soyuz T-5 as mission Engineer.
He was born in Moscow, Russia in 1942, which would of been tough, because of the War and everything and Stalin.
He logged 211 days in space, which he wrote into a book Diary of a Cosmonaut: 211 Days in Space.
He stayed on the Russian Space Station Saylut for 211 days.
He retired from the space program in 1993.

He's still alive to this day he's 65 years old.

Hey guys, its Georgi Beregovoi the Russian Cosmonaut, flew on Soyuz 3, The soyuz 3 mission was was supposed rendevous and dock with the unmanned soyuz 2, but that failed. The Russian Space Agency say it was finally Georgi's bad piloting. Which i think is quiet wrong, the astronaut or cosmonauts will do everything they can to save the mission, there's no such thing as bad piloting in space exploration.

But he sadly died in 1995 after a battle with heart disease, may he rest in peace.

There are many examples of bad piloting in space. Especially on the Russian side, which rely heavily on mission control to know about the state of the vehicle, but have only the minimal needed trust into the astronauts. The collision of a progress with Mir for example was one such example - it happened while the progress was under remote control by an astronaut.

Astronauts make errors, too.
