Maybe the barge is the issue?

The adapter is the wide-base white cone below, which interfaces the 2º stage to the payload.That is just an adapter
To confirm that, you can turn on the force vectors in Orbiter and see if the thrust is greater than the weight.She is too heavy also as she sinks at launch
