IMO, this type of disaster at Starbase was only a matter of time. Poor quality assurance and burnout culture. Glad it didn't happen with a full stack.

IMO, there's a decent chance of happening, as they might just launch the next v2 Starships without static firing them.

I think they should go for a Space Piñata concept. Just make the StarShip shell out of Starlinks and have them break apart in orbit.

I think the "breaking apart" thing happens on its own as a feature of the Starship. They call it a "RUD", which stands for: Success :P

TL;DR:
  • B14-2 exceeded methane transfer tube structural limits on descent due to the higher AOA test, prop mix and kaboom. Found their limit, won't try again until V3;
  • S35 developed a leak in the methane tank pressure diffuser 3min into burn, caused nosecone pressure buildup, after SECO excessive venting of that pressure caused temporary attitude control error, and liquid methane backflow eventually triggered passivation and control loss. Door likely failed due to high pressure inside;
  • S36 had a nitrogen COPV pop due to unscreened/undetected damage, will run future COPVs at lower pressure, add external covers and introduce new non-invasive quality control and proof tests before loading propellant.
