So, closing VS and returning to GitExtensions, above the selected commit there should be one with the message "Fixed LuaInterpreter compilation with default options turned off (#532)", which I think fixes that issue. Right-click in it, "Checkout this commit" and check the "Force" checkbox, and the green box should now be in that commit.
Then returning to VS, open the CMakeLists.txt file and turn OFF XRSound and others, leaving just D3D9 ON, then right-click "Delete Cache and Reconfigure" and then "Build".

Bypassing lua and its problems:
Open file Src\Plugin\CMakeLists.txt and comment (using #) the lines with "LuaConsole", "LuaMFD" and "ScriptMFD", which should be 2 of each.
Open file Src\Module\CMakeLists.txt and comment the LuaScript line.

Then same old "Delete Cache and Reconfigure" and then "Build".
 
That yielded complete success in the build phase I've replaced the original release files with the newly built debug versions.

(I should have been more clear, the attach to process should have been performed on the Orbiter project, not on SSV... but the Orbiter files still had the debug info and VS got to the correct source, so no problem.)

How do I check for that?
The call stack is the list of functions called, shown in the lower-right window.
So first get a print or copy the text of that call stack window, to see how it got there, then we'll start looking at the values of the variables.

ill give this a try when im back at my apartment, but what is weird is i cant replicate the issue on a separate orbiter install. i thought it was something i installed that edited the elevation maps on the west coast but i tried installing that and its still fine.
 
From what I see, the derailment seems to start somewhere in the early part of the function Scene::RenderMainScene(), or possibly earlier than that... but I still have no idea of the cause.

I've rechecked my code, and other than some useless casts left over, everything in the 2016 -> 2024 conversion seems OK: pens, fonts, etc, and most* surfaces, either get created at the start or I throw an exception.

*) the surfaces for the ADI tapes and CCTV monitors are not protected when created at the start...🤦‍♂️ the CTD on loading the pad cannot be explained by the monitors, as they are off and the surfaces are not in use, but the tapes could technically cause it, but I doubt it as there are so many calls to paint in them, that I'd say it would crash right there, and not later deep in D3D9. As for the later CTD, I don't think these surfaces would cause it...
I'll add the missing protections to all these, but I'm not confident that this will fix the issue.

Done. CTD at the start with a message, or "[ERROR]" messages in the log along with missing displays should appear instead of a "simple" CTD.
 
I would suggest the following idea: If an engine exceeds a critical limit (turbo pump temperature, combustion chamber pressure), simulate the automatic shutdown of that engine by the GPC. This would force it into an abort mode (RTLS, TAL, ATO, AOA). Ideally, try to simulate the abort modes; that would be my idea to make it a bit more realistic.

I've been doing some back reading and just noticed this post. (It probably wasn't visible at the time, as it was your first and needed moderator approval to show up)

About the aborts, many things are missing for that, namely ascent logic, but also a better OMS/RCS to allow for prop dumps during ascent. Better OMS/RCS will then need better logic to handle the orbit segment of the flight. So instead of making everything at once, I'm working first on the OMS/RCS and orbit for v2.0 (and it's already a massive rework) and then later I'll work the ascent, and aborts, as I'll have all that is needed for those.

As for the SSMEs failing, actually it is the SSME controller that shuts down an unhealthy engine. If you kill the power to the controller (yellow switches on panel R2), there is a sort of "dead-man switch" in the engine that will cause it to shutdown. The GPCs will have no idea what happened, except that the engine no longer sends data, and you need to press the correct Shutdown PB on panel C3 to let the GPCs know that the engine is gone. The failure handling in the SSME controller is more or less functional, but as there are no failures yet, it isn't exercised. You can always manually kill APUs, EIUs, MDMs and create an hydraulic lockup, command/data path failure, display and/or switch failure, and try to work around the situation... but there are no automated aborts yet. 🤷‍♂️
 
Ive noticed something with the IUS when looking at a real deployment checklist. The IUS DPY ENA switches seem to deploy the IUS from the ASE, despite there being the IUS DPY switches right next to it. Am I missing something about how the system works or is it something else? I'm referencing the ISU Deploy checklist from STS-93 (https://www.ibiblio.org/apollo/Shuttle/Payload/STS-93 IUS Deploy Checklist.pdf)

Yep, the Pri deploy switch was wired backwards, so as soon was the deploy enable was turned on, -> deploy. 🤦‍♂️ It is fixed now.
